Make A Voluntary Statement
If you have been contacted or interviewed by an officer from the Wales Police Department, and asked to complete a Voluntary Statement, you can do that by clicking on the link below. You can download the pdf, print the first page (and additional pages if you need them), and write your statement.You can then either mail it to the police department, place it in the drop box at the Town Office, or scan it and email it to the officer, by clicking on their name on the Department webpage and emailing it to them as an attachment.
