ROAD RENAMING PROCEDURE

Applicant's Instructions

Obtain two (2) certified lists of abutters from the Office of the Board of Assessors.

Complete the Planning Board's Road Renaming Form.

Submit the form to the Fire Department for approval.

Mail all abutters (certified with the return receipt to the Planning Board) authorization forms stating that they
agree to the name choice(s) indicated on the Road Renaming Form. Acceptance by two thirds of the abutters is
required to proceed with changing the name of a road.

A check payable to the Town of Wales in the amount of $300 along with the Fire Department approved Road
Renaming Form shall be filed with the Town Clerk.

Submit the completed abutter authorization forms, the Road Renaming Form and one (1) copy of the certified list
of abutters to the Code Enforcement Coordinator who will schedule a public hearing on the proposed road name
change.

The Planning Board holds said public hearing after proper posting and notification to all abutters.

The Planning Board, by majority vote, issues an order to rename the road. The name will be approved in a written
order, signed by the Planning Board Chair. The Code Enforcement Coordinator will file a copy of the order with
the Hampden County Registry of Deeds. Confirmation of filing will be provided to the applicant, the Board of
Assessors and the Town Clerk.

Provided the Planning Board votes to approve the renaming of the road, the applicant shall return to the Fire
Department a copy of the Road Renaming Form signed by the Planning Board Chair along with one (1) copy of the
certified list of abutters. The Fire Department will coordinate the required notifications to public safety agencies
and the appropriate utilities. The Fire Department fee for this change is $25.

Following name change approval, the applicant shall approach the Highway Department to erect appropriate
signage indicating the new name. In the case of a private road, the applicant shall reimburse the Town of Wales
for the cost of the signage to be installed by Highway Department personnel.

The Code Enforcement Coordinator shall provide notification to all abutters of the Planning Board's decision.
